2025 Performance Improvement Series - Prevention of Medication Errors at Transitions of Care

Medication errors at transitions of care often stem from miscommunication, lack of coordination, incomplete medication information, and insufficient patient education. These breakdowns can result in serious consequences, such as adverse drug reactions, delays in treatment, avoidable readmissions, and, in severe cases, patient harm or death.

 

This program will address the root causes of medication errors and implement evidence-based strategies to strengthen processes and improve patient safety.
