Overview: Appropriate pharmacotherapy for heart failure (HF) improves outcomes, including reduced mortality, readmissions and hospitalizations, while also improving quality of life. To effectively manage heart failure it is crucial to have a collaborative interprofessional team of physicians, advanced practice providers, nurses (RNs), pharmacists, social workers and other healthcare professionals. The inclusion of a pharmacist on the team is important to facilitate guideline-directed medical therapy (GDMT) medication management. Learn how two organizations implemented a multidisciplinary approach to improving guideline adherence and patient outcomes.
