Aminoglycoside Guidelines and Pharmacist-to-Dose Policy for Adult Patients

This continuing education activity will describe various dosing strategies of aminoglycosides based on patient characteristics. It will identify appropriate monitoring strategies including the use of pharmacokinetic calculations to conduct therapeutic drug monitoring. Finally, it will describe the pharmacists’ responsibilities in dosing and monitoring adult patients as part of the institution’s ‘pharmacist to dose’ policy.

  1. Demonstrate methods for determining the initial dose and frequency of extended-interval, critically-ill, and other aminoglycoside dosing strategies.
  2. Use appropriate monitoring strategies, including pharmacokinetic calculations, for extended-interval, conventional, and critically-ill aminoglycoside dosing strategies.
  3. Review the pharmacists’ role and responsibilities when consulted to dose and monitor aminoglycoside therapy in adult patients.
