This program will focus on the strategy Spectrum Health took to develop and implement clinical pathways that reach across the continuum of care.  Spectrum Health was selected because they have built an extremely purpose driven process that is prioritized based on the organizations’ needs.  They have over 50 pathways in place that were built with leadership support and front-line staff input.  Panelists will showcase their Heart Failure pathway and discuss how it focuses on the patient’s needs across all areas of service within their system.
