This activity will provide members with insights from others who have tackled these issues with success. It will also provide specific examples of successful outcomes where appropriate. The Vulnerable Patient Populations Network Virtual Series will provide a forum for leaders of various professional disciplines to discuss successes and challenges and engage in discussion with leaders from hospitals, health systems and community partners who will bring evidence-based knowledge to the discussions.

2. Describe how organizations are developing strategies to identify and address social determinants of health needs for vulnerable patient populations.
