PSO 2023 HRO AND Safety Education Series: Conducting an effective cause analysis

Attendees will gain knowledge on leading practices for triaging events and prioritizing events for improvement, when to conduct RCAs and other types of analyses (common cause, apparent cause, PDSA, etc.), how to disseminate learnings and action plans, and monitor compliance to action plans and improvement.

  4. Describe how organizations develop, monitor, and oversee action plans to ensure their effectiveness and sustained improvement.
