2024 Reflection - T77 - Innovating Together: A Multidisciplinary Care Approach to Reducing Hypoglycemic Events - On Demand
Overview: The Quality and Accountability Scorecard incorporates a hypoglycemia metric, which defines a blood glucose ≤50 mg/dL for a patient receiving insulin as preventable harm. Two organizations leveraged multidisciplinary teams to improve the hypoglycemia rate at their centers through appropriate medication use and reduction in practice variation. Learn about their clinical decision support, standard operating procedures and use of pharmacy data that resulted in improved outcomes.
