2024 Reflection - T13 - Seamless Transitions: Enhancing Patient Outcomes, Reducing Readmissions and Improving Care Delivery - On Demand
Maintaining continuity of care as patients transition to different settings can be fraught with obstacles. In some cases, gaps in these transitions can lead to avoidable readmissions. Join this session to learn how an organization leveraged data to optimize care for multi-visit patients, scheduled follow-up appointments with patients prior to discharge and established a post-discharge clinic. Key metrics and outcomes will be highlighted, as well as the dashboard leveraged for delivery of these key performance indicators.
