Vizient Member Networks Spring Meeting – Unlocking Access, Advancing Care - From Measures to Margins
This interprofessional capstone session focuses on health system strategies to address access and capacity challenges across the continuum of care. Participants will examine how organizations are redesigning ambulatory, acute, virtual, and home-based care models—leveraging data, technology, workforce redesign, and community partnerships—to better align demand with the appropriate setting of care while maintaining safety, quality, and patient experience. The session emphasizes practical, leader-ready approaches for scaling innovation and sustaining performance amid ongoing operational and financial pressures.

Learning Objectives
At the conclusion of this activity, participants should be able to:
1. Describe key drivers of access and capacity constraints and summarize implications for patient experience, safety, quality outcomes, workforce sustainability, and affordability.
2. Identify at least three scalable strategies to expand capacity across the continuum (e.g., ambulatory redesign, hospital-at-home/home-based care, AI-enabled triage/decision support, community partnerships) and select appropriate use cases based on patient and system needs.
3. Outline governance and measurement approaches that support reliable, high-quality care across evolving sites of care, including monitoring of outcomes, safety, and patient experience.
