Patients with complex social and medical needs have a disproportionate utilization of health care, especially emergency department visits and medication costs. Current practice is to manage high-cost, high-need patients through various provider appointments and fragmented care. Furthermore, patients experiencing homelessness and/or social disparities have further difficulty managing various healthcare providers and different disease states. This CE presentation will present new and existing data for the utilization and patient experiences of A-ICUs. This will help pharmacy staff and health care professionals to make appropriate decisions for their complex care patients regarding their chronic disease management.
