Vizient Southern States - Diabetes Management Collaborative of the Year
Education will be provided re: the importance of addressing medical nutritional therapy in improving and even eliminating type 2 diabetes. Participants will hear from subject matter experts in the field of diabetes awareness including a case study presentation on how one organization has implemented a collaborative approach to managing their diabetes population utilizing standards of practice with pharmacists. This program will educate and motivate participants to action, through knowledge transfer and networking opportunities.

- Outline the development and impact of a diabetes education and free supply program pilot for health system employees
- Discuss the potential benefits that embedded pharmacist offer providers and patients with diabetes
- Define the development of an embedded pharmacist pilot program
- Identify barriers of care and their effect on caring for diabetic patients
- Describe the evidence for medical nutritional therapy in the management and reversal of type 2 diabetes
- Describe how continuous remote patient care can be utilized to manage chronic conditions like type 2 diabetes and expand access to care in remote geographies.
- State factors that allow for sustained behavior change in diabetes reversal.
- Recognize member diabetes program improvements, accomplishments, future opportunities and sustainability plans
