ISMP Highlights for Action: Recent Medication Safety Concerns
This activity will identify priority medication safety findings and recommendations that have been identified from events submitted to ISMP and published in their Acute Care Medication Safety Alert! newsletter in the fourth quarter of 2025. Participants will then be able to immediately identify and address the risk associated with these issues to reduce the frequency of related medication safety errors and improve patient outcomes.

Learning Objectives
Pharmacists
At the conclusion of this activity, pharmacists should be able to:
- List three errors or hazard examples that have been reported to the ISMP Medication Safety Errors Reporting Program (ISMP MERP) and published during 4th quarter of 2024.
- State three strategies that can be used to identify and reduce the risk of current medication events.
- Describe how the reporting of unsafe conditions or actual events that could lead to harm can be used regularly to support improved outcomes as part of a robust medication safety program.
Pharmacy Technicians
At the conclusion of this activity, pharmacy technicians should be able to:
