This program will highlight the results of Vizient’s Health Equity Benchmarking Survey, the regulatory requirements around Health Equity, and the successful approaches of two prominent healthcare organizations. Yale New Haven Health System has achieved notable results by utilizing Clinical Pathways to drive outcomes and improve health equity. Similarly, the University of Alabama at Birmingham (UAB) has made a significant impact by enhancing care delivery and addressing diabetes through community partnerships.
