Digging into Digoxin: An Evidence-Based Review of Its Role in Past and Present Therapy
Digoxin, a cardiac glycoside, is often used when first-line agents are ineffective for treating heart failure and atrial fibrillation. While digoxin was historically a popular treatment option for heart failure, concerns over its efficacy and safety have limited its use in the present day.
This activity will provide essential knowledge of digoxin, including its indications, mechanism of action, pharmacokinetics, and a review of the current evidence to foster well-informed decisions and enhance the overall management of digoxin. This will include an interactive component where participants will complete knowledge checks to assess their learning throughout the activity.

Learning Objectives
At the conclusion of this activity, Pharmacists should be able to:
- Describe the mechanism of action, pharmacokinetic properties, and therapeutic serum concentration range of digoxin.
- Evaluate the evidence supporting or opposing digoxin use for symptom reduction in HF and rate control in AF.
- Apply current clinical practice guidelines to determine appropriate patient selection, dosing, and monitoring of digoxin in HF and AF.
At the conclusion of this activity, pharmacy technicians should be able to:
- Identify available dosage forms, strengths, and routes of administration of digoxin
- Recognize digoxin as a high-alert medication and apply appropriate verification steps during medication preparation
- Describe when to escalate potential discrepancies or concerns related to digoxin to the pharmacist
