2021 PI Collaborative Series: High Utilizers: Identification and Care Management – On Demand

During this series, we will look at issues facing health care organizations, such as improving staff well-being, the identification and care management of multi-visit patients, improving capacity management, clinical documentation improvement and patient transitions. All topics contribute to performance improvement.
