2026 Impact - W95 - Strengthening Care Transitions To Reduce Readmissions - On Demand
Reducing readmissions for complex conditions requires coordinated, multidisciplinary approaches that extend beyond discharge. Allegheny Health Network brought together a networkwide task force to address congestive heart failure (CHF) readmissions through structured care transitions, integration of social determinants of health and strong partnerships across care settings. In parallel, a standardized chronic obstructive pulmonary disease (COPD) pathway at Houston Methodist West Hospital aligned interdisciplinary teams around evidence-based inpatient care, patient education, medication optimization and proactive follow-up. Together, these efforts demonstrate how consistent frameworks and collaboration across the hospital-to-home continuum can improve continuity and reduce readmission rates. Join the session to learn how these models were implemented.
