2026 Impact - W75 - Reducing Cardiac Catheterization Laboratory Overtriages by EMS: A Quality Project - On Demand
An ST-segment elevation myocardial infarction (STEMI) is a life-threatening heart attack requiring rapid identification and treatment in the cardiac catheterization laboratory (CCL). Diagnosis and field activation of the CCL by emergency medical services (EMS) can improve treatment times. However, the CCL was too often overtriaged (inappropriately activated) by EMS — negatively impacting patients, staff and resources. We worked to reduce overtriages without affecting treatment times, encountering change resistance and concerns about causing potential harm. Our findings demonstrate that a multidisciplinary team applying the Define, Measure, Analyze, Improve and Control framework can effectively identify and mitigate the root causes of overtriage. A deliberate approach to change is essential to ensure a high level of care while preventing delays.
