2026 Impact - W67 - Turning Age-Friendly Goals Into Everyday Practice - On Demand
Delivering age-friendly care at scale takes more than intent — it requires systems that make it reliable. At North Shore University Hospital, an interdisciplinary performance improvement collaborative uses shared metrics and structured working groups to advance the 4Ms Age-Friendly framework and improve care for hospitalized older adults. At UofL Health, the same principles are embedded directly into the electronic health record (EHR) through automation, clinical decision support and standardized workflows to hardwire consistent practice. Together, these efforts show how aligning data, teams and technology can translate age-friendly goals into everyday care — bringing consistency to a complex patient population. See how this work can take shape in your organization.
