2026 Impact - T43 - Two Layers, 1 True North: Transforming Clinical Quality Through Data-Driven Governance - On Demand
Yale New Haven Health identified a widening gap in how quality performance was measured and understood, including strong ambitions constrained by fragmented metrics, inconsistent benchmarks and variation hidden within service lines. By pairing unified enterprise objectives with a hospital-level quality performance management framework, the health system gained greater visibility into performance trends and the ability to respond more effectively. This two-layer model uncovered opportunities traditional benchmarking had obscured and strengthened alignment across governance, analytics and interventions. The approach contributed to improvements in healthcare-associated infections (HAIs), mortality and Patient Safety Indicators (PSIs), while supporting performance across multiple national quality, safety and regulatory reporting programs. Attendees will learn a practical model that can be adapted and replicated within their own organizations.
