2026 Impact - P161 - Turning the Ship: Making Waves in Expected Mortality - On Demand
In our first year with Vizient data, the organization identified critical discrepancies in observed outcomes versus risk-adjusted performance. Internal reviews confirmed documentation and capture issues. With executive sponsorship, we launched machine learning-driven clinical documentation improvement (CDI) nudges at two hospitals to prompt capture of clinically present comorbidities, while implementing targeted education on severe sepsis/septic shock and high-impact diagnoses at another site. As a result, sepsis mortality index decreased from 1.56 in January 2025 to 0.89 by October 2025. One specific CDI nudge test hospital showed an improvement from 13.84% to 33.88% in fluid and electrolyte disorder documentation, as well as positive physician feedback.
