2026 Impact - P133 - From Hospital to Home: Optimizing Oncology Transitions Through Predictive Analytics - On Demand
Discharge to home is the ideal outcome for oncology patients, yet demographic and clinical barriers may delay or prevent a safe transition. To address this challenge, Robert Wood Johnson University Hospital and Rutgers Cancer Institute developed a predictive tool to identify patients at risk for nonhome discharge early in their hospitalization. In collaboration with the Oncology Transitions Operations Committee, including transitions specialists, oncologists, hospitalists, physical therapists and case managers, targeted interventions were implemented with a focus on mobility and access to home care. This data-driven, team-based approach significantly improved the rate of home discharges.
