2026 Impact - P109 - Breaking the Sitter Cycle: A Smarter Approach to Patient Safety - On Demand
Hospitalized patients ages 65 and above face elevated safety risks from medication effects, increased length of stay, altered mental status and other complications. Traditionally, patient care technicians (PCTs) provide constant observation (CO) at the bedside. This session describes how an urban academic medical center transformed its observation culture by implementing an innovative four-tier model that integrates the Age-Friendly 4Ms (What Matters, Medication, Mentation and Mobility) framework, virtual observation, and nonpharmacologic interventions. These efforts resulted in a 21% reduction in CO utilization, a 20% savings in overtime and improved PCT availability — all without adding additional full-time equivalents or capital investment. This project demonstrates how standardized protocols and leadership accountability can optimize safety while improving resource utilization.
