Sleep plays a critical role in cognitive function, metabolic regulation, and overall health, yet hospitalized patients often experience fragmented sleep due to disease burden, environmental noise, frequent monitoring, and nighttime medication administration. Older adults are particularly vulnerable to sleep disruption, as age-related changes reduce deep sleep and increase nocturnal awakenings. In the inpatient setting, routine administration of non-urgent medications during patient sleeping hours can contribute to unnecessary sleep fragmentation. Emerging evidence suggests that clustering medication administration during waking hours may improve sleep quality and potentially reduce delirium incidence. Research predicts that delirium affects up to 50% of hospitalized older adults and is associated with increased morbidity and mortality. However, limited data exist linking medication administration timing with sleep disruption and delirium risk. This study seeks to characterize the frequency and patterns of oral medication administration between 0100-0500 and evaluate how medications can be retimed to limit sleep disruptions.

Background:  End stage renal disease (ESRD) is defined as the final stage of chronic kidney disease (CKD) where kidney function is unable to sustain life without dialysis or transplantation. Patients with CKD are at high risk for developing hypertension. In an observational study looking at the pre-hemodialysis (HD) blood pressure variation, risk of all-cause mortality, and major adverse cardiovascular events (MACE), greater pre-HD systolic blood pressure (SBP) variation was associated with an increased risk of primary outcomes. These risks emphasize the importance of improved blood pressure control in HD patients to decrease complications. Another comparison of whether holding versus administering antihypertensive medications before HD did not prove noninferiority. This data indicates that managing HD patient’s blood pressure is essential for reducing adverse outcomes.  

Glucagon-like peptide-1 receptor agonists (GLP-1 RAs) have emerged as powerful therapies for type 2 diabetes, obesity, sleep apnea, and chronic kidney disease, offering meaningful improvements in glycemic control, weight reduction, and cardiovascular outcomes.1,2 There has been a steady increase] in prescribing of GLP-1 RAs beyond the diabetic population in the United States. New GLP-1 RA starts in individuals has increased more than 700% between 2019 and 2023.3 Despite these dramatic trends, there is limited insight into how GLP-1 prescribing is evolving within the Medicare and Medicaid population. The purpose of this study is to characterize national prescribing and utilization of GLP-1 molecules.

Purpose: Our emergency department implemented a cellulitis treatment algorithm including dalbavancin on August 1st, 2023. We sought to assess adherence to criteria for use and utilization overall.
