PSO 2023 HRO And Safety Education Series: Improving RCA outcomes: Including the involved staff and obtaining external expertise

Attendees will gain knowledge on the benefits of involving the health care team involved in the adverse event in the RCA meetings, selection of solutions and the action plan. Attendees will learn when and why it is important to seek external expertise in RCA.

  3. Describe situations in which obtaining external support is needed to obtain an objective, thorough and credible analysis.
