Learning from peers, experts and those who have discovered and uncovered best practices can shorten the time it takes to solve problems in an organization. The 2021 Vizient Clinical Connections Summit is designed to give our members the opportunity to share their knowledge, experience, lessons learned and take home real life, actionable takeaways they can implement.

Based upon the 2020 member topics survey, the Summit sessions and posters provide a through-line to successfully tackle some of the toughest challenges members currently face. Learner takeaways includes proven strategies and tools to enable change and evolve care models. This education content illustrates the depth, sophistication, and impact of Vizient members’ work to improve patient care quality, safety, experiences, effectiveness, outcomes, and efficiencies.

• Address identified health disparities gaps by partnering with community and civic organizations to direct resources that improve post-hospital discharged patients’ survival and ability to thrive in their community.
