2026 Impact - W72 - Implementing Social Work-Led, Preoperative Discharge Planning in an Ambulatory Setting To Reduce Inpatient LOS - On Demand
Academic medical centers (AMCs) manage high-acuity surgical cases that often require postacute discharge planning. Preoperative planning in ambulatory settings has shown to improve length of stay (LOS) outcomes for AMCs facing throughput and capacity pressures. In these settings, licensed clinical social workers identify psychosocial barriers, reduce length of stay through proactive discharge planning and prepare patients for postoperative needs. Building on successful models in urology, orthopedics and plastics, this approach was implemented in a cardiac surgery clinic. Outcomes indicate strengthened care coordination and better alignment of preoperative planning with postoperative needs, supporting safe and efficient discharge pathways.
