Clinical Documentation Integrity (CDI) plays a critical role in supporting accurate patient records, appropriate reimbursement, and quality reporting. This self study continuing education activity introduces nurses and other health professionals to the foundational concepts and practices of CDI. Participants will learn how complete and precise documentation reflects the true severity of illness, supports continuity of care, and strengthens data integrity across the health system. The activity provides a step by step understanding of key CDI principles, documentation best practices, and the role of collaboration between clinical and coding teams in improving patient outcomes and organizational performance.

Learning Objectives

 

Upon completion of this continuing education activity, participants will be able to:
 
  • Define the purpose and importance of clinical documentation integrity in health care settings.
  • Identify common documentation elements that influence coding accuracy, quality metrics, and reimbursement.
  • Recognize documentation gaps and inconsistencies that can impact patient outcomes and organizational reporting.
  • Apply strategies for improving the completeness and clarity of clinical documentation using current best practices.
  • Explain the role of nurses and interdisciplinary team members in supporting accurate and compliant documentation.
  • Demonstrate understanding of how effective CDI practices contribute to regulatory compliance, data accuracy, and patient safety.
  • Collaborate with respiratory therapists and other members of the health care team to optimize patient outcomes and prevent complications.
  • Educate patients and caregivers on maintaining adequate hydration and understanding how medications may affect fluid balance.
  • Collaborate with the health care team to develop and carry out individualized care plans that address both medication and hydration needs.

Many state licensing boards and credentialing bodies accept certificates of participation from accredited CE activities to meet CE requirements for license renewals and re-certification.
