The purpose of this activity is to develop the foundational knowledge needed to describe what transitional care is and how to evaluate the efficacy of transitions of care programs and models. It will review several existing programs and identify metrics utilized to assess their success. Additionally, it will examine the services pharmacists can offer during the process to help ensure safe and effective transitions across phases of care. This activity will provide knowledge and program examples to help ChristianaCare identify ways to bolster its own transitional care practice.
