Thank you for your interest in planning an accredited Continuing Education activity with Vizient’s CE Department. 
As an accredited provider of continuing education (CE) Vizient is dedicated to ensuring that all educational content is clinically valid, fair, balanced, supports safe, effective patient care, and is free from commercial bias.
To ensure your proposed CE activity meets all the necessary requirements for accreditation, Vizient’s CE department must be integrally involved in the planning and implementation of the accredited CE activity. 
Please note:
  • The Request for CE Credit must be completed prior to the start of the planning of your CE activity. This form can be completed at any time, as far in advance as possible, before the planning of your activity begins.
  • You must complete all required form fields before submitting this form. You will be able to save the form as a draft and return to complete the information later.
  • If your target audience consists of two or more professions, you must identify someone from each target audience to serve as a member of the planning committee.
  • All individuals identified as activity planners will receive an email instructing them to complete a Disclosure of Financial Relationships.
  • Once we have received and reviewed your Request for CE and Disclosure of Financial Relationships of you will receive an e-mail detailing the next steps.

Course Director

Course Coordinator
