Under the Health Insurance Portability and Accountability Act (HIPAA), Protected Health Information (PHI) is any information related to an individual’s health condition, the provision of or payment for health care, that could be used to identify the individual.
 
It is the presenter’s responsibility to ensure that no PHI is included in their presentation. Please carefully review the information below and complete the attestation.
 
Examples of PHI Include:
 
Personal Identifiers:
  • Names
  • Social Security numbers
  • Telephone numbers
  • E-mail addresses
  • Fax numbers
Health and Record Identifiers:
  • Medical record numbers
  • Health plan beneficiary numbers
  • Account numbers     
Other Identifiers:
  • Dates (other than the year), such as birth date, admission date, discharge date, and date of death
  • Ages over 89 or date elements indicative of age 90 or older
  • Biometric identifiers, such as fingerprints and voiceprints
  • Full-face photographic images
  • Geographic identifiers smaller than a state
  • IP address numbers, web URLs, and device identifiers
This list is not exhaustive. Any other unique identifying number, characteristic, or code may also qualify as PHI.
