2026 Impact - W86 - Postdischarge Clinic Improves Access, Utilization and Continuity of Care Outcomes - On Demand
UC Davis Health launched a Post-Discharge Clinic (PDC) in April 2023 to improve outpatient follow-up for high-risk patients discharged from emergency department (ED) and inpatient settings. Developed through close collaboration between inpatient and ambulatory leadership, the payer-agnostic, nurse practitioner-led clinic ensures access within seven days when postdischarge primary care or specialty access is limited. Leveraging electronic medical record (EMR) risk flags, navigator direct scheduling, telehealth options and transportation support, the PDC significantly reduced 30-day ED and inpatient revisits and helped advance UC Davis Health into top-tier Vizient Ambulatory Quality and Accountability continuum of care performance. This session presents a scalable, replicable model to improve access, equity and operational efficiency.
