2026 Impact - T51 - Don't Sugarcoat It: Impact From a Pharmacist-Managed Diabetes Program - On Demand
Diabetes mellitus affects a substantial portion of the population and is associated with significant complications when untreated or undertreated. Inclusion of an ambulatory care pharmacist within a direct patient care setting enabled an interdisciplinary approach to diabetes management through a collaborative practice agreement. This model expanded access to care, supported providers in meeting quality metrics and improved glycemic control. Over nearly one year, pharmacist-managed care resulted in a mean hemoglobin A1c reduction of -1.54%, demonstrating measurable clinical impact and scalability.
