2026 Impact - T15 - Utilizing Standard Work To Improve First-Case On-Time Starts in the Operating Room - On Demand
First-case on-time starts (FCOTS) are a key indicator of operating room (OR) efficiency and reliability. Delayed first-case starts result in lost prime-time OR capacity, downstream delays and increased labor costs. At our institution, inconsistent workflows and unclear accountability contributed to low FCOTS performance. A multidisciplinary frontline team partnered with executive leadership to design and implement standard work and a defined FCOTS timeline. Using a data-driven approach and staff engagement, FCOTS performance improved from 39% to 74% over five months for this team, enhancing OR efficiency, workforce alignment and patient access while maintaining high-quality perioperative care.
