Our hospital implemented a centralized digital system to enhance patient safety rounds, improving documentation, tracking and resolution of safety concerns. This platform brought C-suite leaders to the front line, fostering direct engagement with bedside teams. It also streamlined supply ordering, reducing inefficiencies and improving resource allocation. Additionally, the project enhanced patient safety by addressing specimen labeling deficiencies in the emergency department and the operating room. A kanban-based dashboard ensures transparency, while integration with The Joint Commission SAFER (Survey Analysis for Evaluating Risk) Matrix supports proactive risk mitigation. Since launch, 266 concerns have been logged with a 71% resolution rate, driving sustainable safety and efficiency improvements.
