2026 Impact - W85 - Reinventing Geriatric Primary Care for an Evolving Capitation Landscape - On Demand
The University of California, Los Angeles Senior Care Model (SCM) is a redesigned primary care medical home for adults ages 65 and older, grounded in value-based care and full-risk Medicare arrangements. Delivered in the ambulatory setting with an interdisciplinary team, SCM aims to improve quality, optimize utilization, streamline navigation and enhance patient satisfaction. Sponsored by the chief medical officer of ambulatory care, the pilot aligns with organizational strategy, focusing on infrastructure, population health, engagement and clinical impact.
