2026 Impact - T25 - Building Hospital Capacity Through Care-At-Home Programs - On Demand
With uncertainty surrounding the Centers for Medicare & Medicaid Services Hospital at Home waiver, health systems need sustainable alternatives to deliver acute care at home. This panel features two academic medical centers — University of Utah Health and Stanford Health Care — who independently developed nonwaiver care-at-home programs that increased hospital capacity using existing reimbursement mechanisms. University of Utah Health Heal at Home program partners with an integrated home health agency and a dedicated transitions of care team, while Stanford's virtual-first model leverages advanced practice providers, 24/7 virtual nursing, case managers and home health partnerships. Attendees will learn practical implementation strategies applicable to their own health systems.
